• Reiki healing is rather an Energy Medicine that is "integratively" used with other diagnostic, treatment, and healing techniques, disciplines, and modalities and generally considered as "Complementary and Alternative Medicine or CAM".
• Your decision to request and undergo Reiki and other multiple-modalities related to healing, therapy, or treatment offered at and by Dia Celeste staff is an informed decision made solely and independently by you.

I hereby give my informed consent to voluntarily and independently participate in an "in-person" or "distant" healing, therapy, class or treatment offered at and by Dia Celeste staff that is intended to provide comfort, relaxation, and / or reduce stress.

I understand clearly and completely that the services provided at and by Dia Celeste staff during these sessions are in no way intended to be a substitute or replacement for my past, current, or future diagnosis, therapy, treatment (including medication and diagnostics), intervention, protocol, interpretation, recommendation, and / or follow-up by / with your medical and other healthcare professionals. I am fully aware that my Reiki or Integrative Pelvic health Class Practitioner will not offer any diagnosis or recommend any Medical Treatment, diagnostics recommendation or interpretation, or prescribed medication.

Furthermore, I understand clearly and completely that I must continue to have regular medical visits, appointments, and/or check-ups as part of my overall personal health care plan; and I should contact my own certified and licensed medical physician / doctor / health care professional for any physical or psychological ailments or concerns that I may have in order to get proper medical advice.

I understand clearly and agree completely that my participation in these sessions is voluntary and that at any time during the sessions I can choose to end my participation. I also understand that I may experience 'self-healing reactions' during the 48 hours following these sessions.

I understand clearly and completely that any information exchanged during any session is educational in nature and is to be used at my own discretion. I also understand that any information exchanged during these sessions is strictly confidential and will not be shared with anyone without my written permission.

Finally, I understand clearly and completely that by providing this informed consent I am assuming full responsibility for participating in these sessions and I hold fully and unconditionally harmless both my Dia Celeste staff and their facility / location where the services are provided.

I agree to all the above terms, conditions, and statements for all my "in-person" or "distant" sessions and certify that the above information is true and correct.
